Walk a defined zone
State the area, date, shift and reviewer. Look at aisles, storage, labels, housekeeping, equipment and pedestrian routes. Use the same interpretation of 0 = not met, 1 = partial, 2 = met.
Worked example
Twelve checks with ten scores of 2 and two scores of 1 produce 22 ÷ 24 = 91.7%. If one unchecked hazard is serious, the percentage does not make it acceptable. Escalate immediate hazards using site procedures regardless of score.
Record evidence
For each gap, write the observed condition, temporary control, permanent action, owner and due date. Store photos only where your organization permits and avoid capturing identifiable people unnecessarily.
Review closure
A responsible person should verify evidence before marking the action done. Repeat observations suggest a system issue: layout, ownership, training or maintenance may need to change.
